Submission Forms


Before submitting a submission form, you are strongely urged to view the CIR movie and to read the rules for submission.

The request must contain:

  1. Your name, address, fax number.

  2. Contact information for the other party; including contact name, address, phone and fax numbers.

  3. Type of agreement (negotiations or grievance)

  4. Please remember that you must notify the other party that forms have been requested!
